In Slack

  1. Create an Incoming Webhook for the channel you want alerts in (Slack → Apps → Incoming Webhooks → Add to Slack → choose a channel).
  2. Copy the webhook URL — it looks like https://hooks.slack.com/services/….

In Formitor

  1. Go to Notifications → Add a destination.
  2. Set Type to Slack, add a Label (e.g. #alerts), and paste the webhook URL.
  3. Choose which sites and events it covers, then Add destination.

Alerts now post to that Slack channel whenever a form fails or recovers.
